Develop Measurable Goals and Timelines

This document is designed to help you develop the goals and timelines you will address with your CDAT. As the initiator of a CDAT, you can share these briefly in your invitation and then in more depth as you start your work.


Identify Potential CDAT Members

Use this document to assemble and consider a list of potential team members. The questions are slightly different for the Single-Driver Group and the Mutual Action Circle.


Develop Operating Guidelines for the CDAT

Use this document as a general guideline to establish the particular way your CDAT will function.


Invite Potential CDAT Members

This document provides examples of invitation letters to send to people you want to ask to join your CDAT.

Information Package Content

We recommend you send each team member a comprehensive information package prior to your kick-off meeting. By giving your team several days to review the package before the kick-off meeting you inform and prepare them.


Meeting Agenda

You can use this document to create an agenda for each meeting you call, whether face-to-face, by telephone, or online.


Meeting Minutes and Action Items

Use this form to take minutes and note action items that come up during a meeting.


Quarterly Review

This form can help you present a summary of activities or learning for the quarter.
