For the Single-Driver Group

The project I am undertaking is to develop and work with a personal Career Development Accountability Team (CDAT). The CDAT is a consultative resource to provide focused, personalized support and direction for my career planning and personal development. It differs from mentoring and coaching by virtue of the collaborative nature of the team.
The purpose of the group is to collectively assist me to [state your goal or goals here]. After I have established the team of [three or four] people, I will provide a more detailed information package and schedule a kick-off meeting. The term of the CDAT will be approximately one year. I anticipate that your involvement will be no more than four hours each month, primarily involving one monthly [meeting, team teleconference, or other form] and one or two personal phone calls or [face-to-face, online] meetings with me. I may have occasion to email you or call you with specific questions or requests, and I commit to be respectful of your time in all cases.
I have established guidelines for confidentiality and other ground rules, and I will present these to the group both before and during our kick-off meeting so everyone has a chance to review and agree.
I believe that becoming a member of my CDAT will provide benefits to you. You will be able to network with the other team members; you will learn in detail how a CDAT works (which will enable you to develop and work with your own CDAT or recommend the tool to others); and you will have access to the learning of the CDAT, which may be of use to you in your own work or personal development.

I would like to initiate and work with a personal Career Development Accountability Team (CDAT). The CDAT is a consultative resource to provide collaborative support. It differs from mentoring and coaching by virtue of the team members’ direct work with one another.
I seek to work with [two or three] other people who are interested in a collaborative model where team members engage reciprocally with each other to support everyone's career planning and personal development. Each member will develop and present her or his own career development plan, receive feedback and suggestions from the members, give feedback and suggestions to the members, and participate in periodic reviews.
The benefit of group collaboration is that members provide support and encouragement and build on each other's work to generate new and creative approaches to adapt to their particular needs and interests. My goals to work on with the CDAT are [state them here].
I believe you would be a strong contributor to the group because of your [explain your reasoning here, such as knowledge of x, experience with y, interest in z…].
After the team of [three or four] people is established, I will provide a more detailed information package for your review. Then we will schedule our kick-off meeting.
